Watch out for hollow trunks hiding under healthy bark

I was doing a routine removal on a big maple in a backyard off Route 9 last month and the tree looked fine from the outside, but once I started the first cut, the whole center was rotten clear up to the crown. A client's kid could have been swinging on that thing and it would have dropped without warning. Anybody else finding that internal decay is way worse than what a simple visual check or even a resistograph reading shows?
