Check your gasket torque before you fire up, watched a door blow in Tulsa last month
Guy I know skipped his final torque pass on a 60 inch manway because he was chasing shift change and the gasket let go at 40 psi, sent the door into a handrail 8 feet away. What is your crew's rule for signing off on a torque sequence, does the lead check it or does the guy on the wrench just mark it done?
60 inch manway at 40 psi is no joke, that door had serious force behind it. Your guy got lucky it hit a handrail and not a body. My rule is the lead signs off, not the guy on the wrench. The person turning bolts is the one in a hurry, and marking your own work done is how that final pass gets skipped. Lead walks the pattern with a paint stick and checks the marks himself before anybody cracks a valve.
