Watched a coworker get locked out of our whole system for 3 hours

Back in 2012 I ran a crew in Sacramento and we all used the same password for everything, the owner's dog name with a 1 at the end. One day the office manager got locked out after a wrong entry and we had no backup way in, so the whole dispatch went down until the owner drove 30 miles to reset it. Have you ever had a shared password blow up an entire day, or was I just the last idiot on the block?
