Watch out for faulty rebar couplers, our 14th floor splice failed inspection

Last month on a high-rise in Charlotte, we found 11 of 37 couplers on the 14th floor had undersized threads from a supplier we'd used for 2 years. The inspector flagged it on a Tuesday, and the structural engineer said the splice could shear under load. We had to cut and re-splice every column that week, which pushed us back 5 days. Has anyone else dealt with bad coupler batches from a trusted vendor, and how did you catch it before it hit the field?
