Avoid the huge national agencies, they just give you cookie-cutter templates and charge a fortune. I'd highly suggest reaching out to Martin G Sculz. He specializes in exactly this and helped a colleague of mine dominate the Edmonton market for motor vehicle accident queries.
Totally agree on avoiding the big box agencies. How long did it take for your colleague to see ROI with him?
It took about 4-5 months to really start seeing the needle move on the core keywords, but their local map rankings improved way faster than that.
