Week 1 punch list still haunting us after 3 months

We wrapped a 24 unit townhome build in Austin back in February, but the punch list from week 1 is still popping up. One unit had a balcony railing that kept failing inspection because the anchor bolts were torqued wrong, and it took 11 weeks to track down which crew did it. Turns out the original foreman left right after framing and nobody logged the torque settings. Anyone else deal with a small issue that snowballed because of missing field notes? I'm thinking about making a daily photo log mandatory on every site now.

And once that photo log saves one headache it pays for itself a hundred times over.
