My client's stubborn milia cleared up after I switched to this one tool
I was about to refer her out for extraction because nothing was working, but then I grabbed a 30-gauge lancet from my kit on a whim. The precision made all the difference and she was thrilled. Anyone have a go-to for those super deep, stubborn ones?

Grabbing a lancet on a whim" seems risky for a client's skin. I'd stick with the proper extraction tools every time.

My buddy went to a new place that did that, and he ended up with a nasty scar. They told him it was just as good.

Proper extraction tools" can fail too, they're not magic. A sharp 30 gauge lancet is still a sterile, single use medical device. If someone knows how to use it right, calling it risky is just fear mongering. The real risk is forcing a dull tool and causing more trauma. Sometimes the standard kit isn't the best answer for every single client.
